The undersigned, an authorized representative of the applicant, hereby authorizes IntelliVoice, LLC and its designated agents to contact the bank and trade references listed above and to request, obtain, and exchange credit and payment-history information for the purpose of evaluating the applicant for credit terms and continued participation in the IntelliVoice Partner Program. This authorization further permits IntelliVoice to obtain business credit reports from consumer/commercial reporting agencies (e.g., Dun & Bradstreet, Experian Business, Equifax Business) as permitted by law.
The undersigned certifies that all information provided is true, correct, and complete to the best of their knowledge, and agrees that IntelliVoice may rely on it in extending credit. A photocopy or electronic copy of this authorization shall be as valid as the original. This authorization is governed by the laws of the State of Florida.
